About Us
The Eastern Devon Population Health Partnership provides a collaborative safe space in which key stakeholders/anchor institutions/VCSE groups from across Eastern Devon can work together to sustainably to improve the lives of people in communities, address inequalities and build community resilience. The forum enables partners to come together to align their strengths and assets to address those issues that impact population wellbeing including – early years support, housing, leisure, transport, skills, antisocial behaviour, education, employment support and the environment.
The forum brings together senior level representation from a range of different organisations who see the value who both to them, and their organisations, of working together cooperatively. The aim is to accelerate joint efforts in area where there is a genuine “sweet spot” where the objectives of partner organisations align. This means that, while the forum has selected one specific focus for its work over the next period, this will not constrain the development of other partnership working within the auspices of the common purpose. In fact, it supports and encourages that to happen.
The organisations represented within the umbrella organisation Eastern Devon Population Health Partnership recognise that partnerships take time and energy to create and are based on trust, relationship building and sharing a similar mindset so we have deliberately given ourselves time to get to know each other. Over time – to ensure sustainability – the relationships we develop based on individual trust will need to be spread across a range of people.

- Eastern Devon Loneliness and Isolation (Social Health) Partnership (name to be confirmed)
- TBC Diabetes (LTC)
- High Intensity Use
Partners are also cognisant that real partnerships evolve out of doing real work together so to focus initial efforts, partners have committed themselves to generating new solutions to addressing the issues faced by people with mental health issues at all ages. The partners are committed to developing new approaches to this issue over the coming months that add value to other existing initiatives.

Our Priorities
- Develop a shared and appropriate language and use across sectors and organisations.
- Use data and community and organisational insight to identify health Inequalities in the Eastern Devon Population.
- Form collaborations of organisations in Eastern Devon who can align goals, resources, and work together to prevent and address challenges.
- Identify and agree the priority groups/issues/places for the stakeholders/the system in Eastern Devon to address together
Our Work (why we want to do this)
- To improve lives of the population in Eastern Devon.
- To increase and strengthen understanding health inequalities in Eastern Devon and share across services and communities.
- To support the mobilisation and empowerment of communities to strengthen their assets.
- Because we want to reduce pressure on stretched services in Eastern Devon.
The Benefits
- Increased information and evidence about Population Health/Prevention/Health Inequalities in Eastern Devon.
- Strengthen the links between primary care, social care and VCSE working in Eastern Devon.
- Provide opportunities for collaborations or organisations and communities in Population Health/Health Inequalities/Prevention work/projects.
- Increase learning for all partners in this space.
- Identify gaps in provision for this population and ensuring these are addressed.
- Large scale positive change for the Population of Eastern Devon.
